To the parents or guardians of future 7th graders, The Tdap is a booster vaccine which continues to protect your child against tetanus, diphtheria, and pertussis diseases they were immunized against as a young child. A Tdap booster is required for students attending school in 7th through 12th grades. Documentation that your child has had the Tdap booster must be turned in before your child can register for the 7th grade. The only form of acceptable documentation is a completed 121 form. Vaccinations are available from the health department or from your local health care provider.
